Admission Information

Youth placed in the Home's care are placed through the Indiana Department of Child Services,  Indiana Department of Education or through the county juvenile court systems. Private placements are considered when a concerned parent, clergy member, guidance counselor or mental health professional contacts the Home on behalf of a child and/or family.

Requests for admission may be made by telephone or letter. At the time of initial contact, a pre-admission interview can be arranged. An application for admission will be sent following this initial contact. This evaluation may take place in the child's home, current placement location or the Home's campus in Lebanon. The decision about admission will be made within 24 hours. An admission date can be scheduled at the time of notification. When a resident's hometown is located some distance from Lebanon, Indiana, the Home may agree to do a direct admission after the interview. Any of the following information is helpful if available: child's social history, any psychological/psychiatric evaluations, any substance abuse assessments, medical history and current immunizations, educational reports and school records and current progress reports from current placement when applicable.

Inquiries should be directed to the Admissions Social Worker at 765-482-5900.
